New patients are encouraged to fill out the online patient information form prior to the first appointment.
You will also find a New Patient Checklist of items to bring with you to your first appointment.
As a reminder, all comprehensive eye exams include dilating eye drops, we encourage you to bring sunglasses with you and if you do not feel comfortable driving after the drops, we also encourage you to bring a driver with you to the appointment.
Comprehensive eye exams also take two hours, so please be prepared to be with us for that length of time.

New Patient Checklist
In addition to the New Patient Forms that can be filled out prior to your first visit, new patients will want to review the following checklist and bring all applicable information with them.
- List of daily medication (dosage and frequency)
- List of allergies
- Name, phone and fax number for your primary care physician
- Location and phone number for your favorite pharmacy
- Insurance identification cards
- Driver’s license or state identification
- Eyeglasses and/or current contact lens information
- Dark sunglasses
